Medial Patellar Desmotomy

Medial Patellar Desmotomy is sectioning of the medial patellar ligament for relief of upward luxation of the patella.
Patellar ligament is the continuation of the central portion of the tendon of the quadriceps femoris muscle distal to the patella, extending from the patella to the tuberosity of the tibia; it is single in carnivores, pigs and sheep and triple in horses, cattle and buffalo; called also patellar tendon.
Care should be taken during sectioning of the ligament, if middle patellar ligament is sectioned then limb becomes nonfunctional and animal cannot walk with that limb.

Procedure

1. Restraning of buffalo calf and giving proper position for surgery.
2. Cleaning the region of surgery; stifle joint with soap.
3. Shaving the surgery region.
4. Injecting the local anesthesia at the site of surgery.
5. Application of povidine iodine for disinfection.
6. Incision of the skin.
7. Haemostatic forceps are used to check blood loss.
8. Gently incision of subcutaneous layer and fascia.
9. Recognization of medial patellar ligament.
10. Sectioning of medial patellar ligament.
11. Stitching of the skin.
12. Application of povidine iodine again for disinfection and protection from bacteria.

This was performed at Institute of Agriculture and Animal Science, Paklihawa Campus by students of B.V.Sc & A.H 5th semester under the assistance of Dr. D.K. Singh in the campus farm.
